Support Letter & Report Request

Please use this form to submit a request for a support letter or report, even if you’ve spoken to your clinician about the request. Reports and support letters can only be requested after attending 3 or more sessions. Please provide a minimum of three weeks' notice for these requests. We are unable to accommodate short notice requests due to this high demand.
